Altus House is a peaceful haven offering solace and comfort to our patients and families.  Most patients choose Altus Healthcare and Hospice as he or she nears the end of life’s journey so that they can live at home.  However, for those who have acute symptoms that require specialized care or whose caregiver is no longer able to care for them at home, Altus House is an excellent alternative.

Altus House provides a warm comfortable setting where patients can receive special care and comfort while surrounded by friends, loved ones and compassionate staff.  Guests are welcome 24 hours a day. With modern eclectic furnishings, Altus House offers two family living rooms for families to gather to enjoy music, TV, conversation and fellowship.   A welcoming room, decorated in earth tones, invites visitors and guests to sample a hot cup of coffee and a delicious sweet treat.  We also have a quiet room where patients and loved ones can retreat for meditation or prayer.

Altus House offers 16 private suites comfortably furnished in a home-like setting. Each suite features:

MEDICAL DIRECTOR:

The Medical Director, along with the hospice care team, develops the patient’s plan of care. He assesses the patient’s needs, prescribes treatments and medications that will offer the patient the best quality of life. He also provides the medical guidance for the interdisciplinary team.

NURSE CASE MANAGER:

The Nurse Case Manager is a registered nurse (RN) who assesses the patient and family needs to develop the plan of care. The Nurse Case Manager coordinates team members to ensure the implementation of the plan of care. He or she coordinates with the Medical Director and/or attending physician to ensure symptom control and pain management. The Nurse Case Manager also provides patient and family teaching as needed.

 
CERTIFIED NURSING ASSISTANTS:

The Certified Nursing Assistant (CNA) provides assistance to patients and families with the personal care needs of each patient. CNA’s provide emotional support and comfort measures to the patient and family. As an integral part of the care team, the CNA reports any identified needs to the Nurse Case Manager.

VOLUNTEERS:

Our trained volunteers provide support to the patient and family in non-traditional roles, such as letter writing, running errands, befriending patients, or simply being in the home or inpatient facility to listen.

CHAPLAINS:

Our chaplains provide spiritual support to patients and their families during their time with Altus. Chaplains may serve as a connection between the patient and community clergy. Altus chaplains are available to meet with members of all faiths to affirm your beliefs. Our chaplains also assist with memorial services and funeral arrangements when needed. Our chaplains are highly involved with our bereavement support groups. Altus bereavement services continue for family and loved ones 13 months after the patients’ death.

SOCIAL WORKERS:

Our social workers provide patient and families assistance in finding financial and emotional resources in the community. They facilitate communication and support families with counseling.

PHARMACY SERVICES:

The pharmacy provides medications related to the patient’s hospice diagnosis and delivers to the home.

HOME DELIVERY OF MEDICAL EQUIPMENT:

The medical equipment company delivers supplies that the Altus Nurse Case Manager deems necessary for the patient’s care and comfort. This includes: hospital bed, shower chair, or bedside commode to name just a few.
