ALTUS HEALTHCARE AND HOSPICE OFFERS SEVERAL LEVELS OF CARE TO MEET THE NEEDS OF OUR PATIENTS AND FAMILIES NO MATTER WHERE THEY LIVE.  WE BELIEVE THAT THERE IS NO PLACE LIKE HOME TO RECEIVE COMPASSIONATE CARE THAT NURTURES THE WHOLE PERSON MIND, BODY AND SPIRIT.

HOME CARE

Most patients served and cared for by Altus Healthcare and Hospice, in more than 20 counties around the Atlanta area, choose to remain in their home to be looked after by a family member or loved ones.  Altus provides support to these primary caregivers who, generally, have never before cared for a terminally ill person.  Often times, we provide care to those who reside in a nursing home or assisted living facility.  We provide support to those facility staff members as well as the patient's family members.

Whether at home or in a facility, our Altus care team develops a care plan designed especially for our patient's need for pain management and symptom control, as well as their emotional and spiritual needs.

Altus will also provide medication, treatment and supplies that are related to the patient's hospice diagnosis. A member of the Altus hospice team makes regular visits to assess the patient and works closely with the patient and family to provide additional care or services when needed all in the home care setting.

CONTINUOUS CARE

If there is a brief occurrence that requires additional care to manage pain or acute medical symptoms, care can be provided on a 24 hour continuous basis to maintain the patient in their home.

INPATIENT CARE

Patients may be admitted for acute medical conditions or pain management to Altus House, our inpatient unit located minutes away from Atlanta's hospital district at 5411 Northland Drive NE, Atlanta, Georgia 30342.

RESPITE CARE

Altus House Inpatient facility offers up to five days of respite care to caregivers who need a short vacation from the many demands of providing daily care.

HOSPICE OFFERS COMPREHENSIVE HEALTH CARE SERVICES FOR PEOPLE WITH A TERMINAL ILLNESS AND SUPPORT FOR THEIR LOVED ONES.

Hospice care is not intended to be curative, but the goal is to provide supportive care for people who have a life limiting illness. Our approach is holistic, meaning we address not only the physical needs of the patient, but also his or her emotional  and spiritual well being. In our "circle of care," we also offer supportive services for the entire family.

Hospice is different from other types of health care because:

  • We utilize an experienced care team who specialize in hospice care.
  • We recognize our patient's emotional and spiritual health as well as their physical condition.
  • We understand that other family members also need assistance and help during this special time in their loved one's life.

THE MISSION OF THE ALTUS ANGELS PEDIATRIC PROGRAM IS TO ENSURE COMPASSIONATE, COMPREHENSIVE CARE FOR CHILDREN WITH LIFE-THREATENING CONDITIONS AND TO CREATE PROGRAMS, WHICH DIRECTLY IMPROVE THE QUALITY OF THEIR LIVES.

Children diagnosed with a life-threatening condition will have access to an innovative, specialized model of care which address their unique needs and that of their families.

ALTUS HOUSE, our inpatient facility, provides respite services when you and your family needs rest and a break from caring for your child at home.


THE ALTUS ANGELS PROGRAM is committed to providing a continuum of care to the family whose unborn child has been diagnosed with a life limiting condition.

Altus Angels'  specialized team will walk with the family from the time of diagnosis, through the pregnancy, the delivery and the death, honoring the baby as well as the family.  The service continues into post-partum, with bereavement support provided for two years after the death of the baby through grief counseling.


Our specialized team respects the wishes and dignity of each child and supports the parent's decisions.

NINA LEONARD RN, MSN, CHPN

Nina is well known in the pediatric community for her work with terminally ill children. Nina has been working as a Pediatric Hospice nurse for 14 years.  She has worked with children from newborn to their 20's with various life-limiting conditions. Nina has extensive background in psychiatric/mental health nursing and pediatrics.

KRISTIN FRAZIER, PEDIATRIC SOCIAL WORKER

Kristin is a licensed clinical social worker who has been working with children and adolescents since 2002. Prior to that she lived in West Africa for nearly three years as a Peace Corps Volunteer, helping to provide care to children and adults with disabilities. As a social worker she has the opportunity to work with families from the point of admission all the way to providing bereavement care for two years following a death.  Each child that Kristin has helped to care for has left an imprint on her heart.
